I did notice multiple versions of jQuery are being loaded, that can cause forms that use conditional logic not to display because it prevents the form scripts from running. The following script should be removed.
For WordPress 3.6 and 3.7 the recommended version of jQuery to use is version 1.10.2 which just happens to be the same version that ships with WordPress and is also being loaded, to rectify this you should speak with your theme developer and ask them to change the jQuery include to use the recommended WordPress method [url removed, login to view] like so
wp_enqueue_script( 'jquery' );
