Need Media Gallery Wordpress Plugin Developed

We need a wordpress plugin created to the attached specifications. This will be for a photo/video media gallery. Screenshots of the final expected product are included in the Zip file.

Plugin must also include an administration component for setting up the media as per the attached specifications. Any changes to the database, etc should be done automatically when the plugin is activated in the wordpress control panel.
