Need a website built for Wellness Center

I need a website built, the main site will be for a Wellness Center and will include an online appointment scheduler, photo album, blog, links, map to the facility, description of services, support center, forum, etc. I also need a main splash area set up that would be under my name (example [login to view URL] I already have the domain name) that will link to all of my different businesses and websites. For example, on this main page, it will link to the wellness center, my client development site, my blog, my candle site, etc. The most important feature I need both of these to do is collect visitor information. I'd like to be able to get the name, address, phone and email of every person that visits the sites, and ideall would like to be able to track how much of a presentation they watch. Should be fairly straightforward and simple to set up, and I'd like to be able to have access to admin pages to make changes and posts. My control panel has joomla, mambo and all kinds of other content management systems we can work with. Would like this to be search engine optimized and perhaps include google adwords or similar.
