Complete The Development of Server Side App

Thank you for reviewing my project.

Summary Background information:

This should be a simple project for an experienced programmer and designer to make changes to and complete the integration of server side app that enables users to complete a survey and receive report based on the result of the survey.

The PHP program code is Laravel and the app is approximately 80% complete. The additions/changes are required following testing of the admin or backend functions.

The user functions/pages are COMPLETED but some minor changes are also required, following testing.

As part of the project, there's requirement for web designer or programmer with excellent web design skills to design direct response style sales and product access/download pages (See attached Word Doc).

More background information:

Users can complete a survey by clicking box with 'a or b' options to answer questions (54 in total). After completing the survey, the user can see the result, based on his answers.

The user will receive report (PDF) via autoresponder (email) that includes information based on the outcome of the survey.

The information generated from the survey is captured in database. The information is retrievable via search function and displayed in various formats (see video below).

REQUIREMENTS::

PLUS:

Install and configure files and database of the app completed by previous programmer.

Complete the functions/pages/edits listed in the attached MS Word Doc.

Develop mobile responsive sales and product pages (See attached Word Doc)

Integrate and configure autoresponder to send reports (output from the survey) to users and to grant access to product pages (See attached Word Doc).

The coding should be structured and clean; malware free and non-usage of open source code/apps that are subject to malware and security issues.

The app (files and database) should be locked down to prevent malware, SQL injection and other threats.

Make changes to the app as required to ensure it is completely functional and bug free.

Provide backup of the completed files/database.

My clients intention is to also develop a mobile version of the server side app following the completion of this project.

I only select experienced programmers with suitable skills and experience to work on my projects. If you don't have the skills and experience, don't waste your time and mine by bidding.

I'll give preference to programmers and expert designers that have also developed mobile apps and web sites

The chosen freelancer will be required to complete an NDA (Non Disclosure Agreement) before starting the project.

My maximum budget for this project is $400.
