Debt Assignment Approval System. Need a website built for users to submit assignments with multiple levels of approval by other users

We are looking for a web application to be written, where a user (1st user level group) could login and submit an assignment through a web form (example attached). The data gets saved to a database and an email gets sent to another user (2nd user level group) notifying them that there is an assignment awaiting for their approval. The user logs in and can "Approve" or "Decline" the assignment. Declining will Notify the 1st user so that they can modify and resubmit. There are 3 levels of approval along the way (4 user level groups - the original submitter "Collector", "Supervisor", "Manager" and "Executive") and on the final level of approval the assignment gets sent to our staff.
We need you to create the site and database to run on our server.
We are familiar with PHP, HTML, CSS, MySQL, Codeigniter and JQuery so these tools are preferred.
We envision:
1. when a user logs in that they see a link to submit "new" and an assignment history to act as the home page, able to be filtered by user or status (approved, declined) and searchable by reference #. maybe with pagination
2. When selecting "new", the user chooses from dropdown which user they are submitting to for approval.
3. The user receiving the requesting email gets a link to go directly to that assignment after entering credentials.
4. Statuses of Approved, Declined, Pending, Expired (for assignments not acted upon) and maybe Archived.
5. some levels of security where users cannot approve their own assignments or not sent to them for approval.
6. Expired or declined assignments can be re-activated and/or modified and resubmitted from history.
7. We need to be able to create update and delete users and their emails as well as the form fields of the assignment form.
8. Each user should have individual login credentials and can be put into one of the user level groups
