I am looking for someone who can create simple forms to input into websites. The form will need empty fields for users to input things such as they:
Name
Address
Contact Phone Number
Comments
A few drop down choices may be necessary but nothing major.
I hope to have simple error handling that can post on the same page as the form stating the errors so a user can fix them.
I am hosted on Cpanel servers and can set up an account if necessary.
Simple quick job for the person who knows forms well.
Please show some examples to make my choice easy.
I hope to spend in the $5 to 10 range.
