MySQL / [login to view URL] - Archive and Delete Script

We need a PHP script that will archive (csv file) and delete records from a MySQL database using mysqldump.exe. The PHP page should accept a date value and archive/delete everything up to that date. Some of the tables do not require archiving.. only delete is necessary. These will run on a Windows server running XAMPP.
Our design idea is to have two sections with a single drop down that contains the table names to process as well as a date field to fill out and a submit button that will initiate the process.
Section one will contain the table names where only delete is necessary.
Section two will contain the table names where archive AND delete is required.
The user would select each table they wish to process, provide the date to delete records for, and process each table that way.

For reference, here are the tables that would be required in this script. (D = DELETE ONLY, A/D = ARCHIVE & DELETE) - All tables have date fields to use as a reference for processing:
