Online Submission Form & Thank you page with some requirement

Need to create an online submission form and thank you page with additional requirement as below

1. Auto Redemption Code generated after form submission

2. Users and admin will both received the submitted form together with the generated code and IP address from the location users submit the form

3. Print, Email, Facebook Share, Tweet function on both submission and thank you page.

4. all submitted info will be store inside an excel file
