PHP Quote Form

Require PHP online dealer quote form. This has to be done, tested and placed on server within 30 days from approval.

Prefer North American developer, but will look at all interested parties.

Details:

-Dealer Login (Htaccess)

-MySQL database (numbered quotes that are dealer specific and cannot be accessed by other dealers).

-Price variables made within the options (please refer to Excel file)

-Quotes must purge after 365 days

The quote form is currently in Excel and available for review
