simple WP or agnostic website with order form

We need someone to put together website in wordpress or individually that has the following order form and features:
First page: Asks if customer is above 18 years of age, if yes continues.
Second page: First order form page asks if women or male
third page: asks what customer likes from couple of options
fourth page: choose your package: package options with link to page with description of each.
description page and payment: in each description page customer is able to order and pay(with paypal). Can use coupon for discount if has one.
Thank you page: Confirms order and gives option to recommend friends and let them get promotions, asks for friends emails.
This order form should be made inside a shopping cart module to take advantage of the features like customer management, newsletters, and coupon management.
We will provide logo and details of each product.
