Convert 3 Basic Paper Forms to a Web Based SQL Database Entry and Reporting application.
IMPORTANT: For a Bid to be considered a small response stating What Language will be used, What technologies will be used (Silverlight, Soap,etc..) Version of Visual Studio, .NET Version, Any 3rd Party tools to be used etc... Although simple application the Visual Apearance MUST be Excelent. The Reporting Features must be Excelent.
Form 1 (PErmit to Work) has 15 Detail Fields and 10 Name Approval Fields. Some Detail fields require multiple selection. 2 Detail Fields link to the other 2 forms. The name Fields require approval by signing in with user/pass to fill as an electronic signature.
Form 2 (Energy Isolation) has 5 Detail fields but each detail field can have up to 5 records each. (Sections 2.1 & 2.2)
Form 3 is linked to form 1 and is mostly text fields and name fields. The Second page is selection fields that can have multiple selections.
All Selection Field in All 3 forms should have a configuration form allowing each to add/edit entries available in other forms as dropdowns or popup check box form for multiple selection.
Security is Login with 4 levels of Form Security. View, Report, Edit, Add.
Each of the 3 Forms should be able to print in a pleasing orderly way.
A BAsic Reporting Page(s) are required.
List all Open PErmits.
List All PErmits FromDate thru ToDate
List Permits by Creator
List Permits by Area/Location
OR more complex allowing to Add more criteria dynamically or use more filters.
Export to Excel required for All reports.
Visual Apeal and ease of use is important.
TO be considered, Descriptions on How the requirements will be developed is required. How will you handle adding multiple records in database tied to a single field in a form.
