Need Script For Reformatting Data

I need a script that will take data from 1 spreadsheet and load it into another spreadsheet.

Please take a look at the attached zip file with 2 spreadsheets.

In the before example you will see how the data is downloaded.

In the after example you will see how I need the data to look.

I only need the 2 fields (email address and first name) to transfer from one sheet to another. But there are some variables I will need added.

Please take a look and advise with questions or comments?
