In Office 2003 -Create 3 Template Forms.
1. Allow forms to be printed
2. Allow new records to be created
3. Allow existing records to be reviewed/edited/printed
There is only hard copy (300dpi) scan to work from.

On these images - Where the Yellow highlighter is marked on page - This is where the form fields go. The user will be able to type into these fields. The rest of the page should be locked from editing.
The 'c' means center aligned.
The 'R' over the numbers comlumn means right aligned.
