osc contrib help and site fix

Hi there I am looking for someone to help me complete my osCommerce store,
I have two main requirements for this project
1)Shipping Checkout Contrib.
I have found a site with the exact osc contribution which I require, but am unable to locate the contrib which has been used
I require someone to help find and install the contribution for me
the contribution does the following
When more than 5 items are placed in the shopping cart, there is no shipping cost Shown (TBC), below this a contact form which enables the customer to contact us with details for us to provide the shipping total, if any quantity below 5 is added standard shipping is applied
i require the above setup for my site

2) Correcting Sql Error on the store page
1064 - You have an error in your SQL syntax on website
