You will primarily use the site [url removed, login to view] to obtain all the information required with the exception of the e-mail address, in which you will have to follow the link to each individual site to get a contact e-mail address. ? Not all of them will have an e-mail address as it directly linked to a form. ? Not all of them will have first names, some only a practice name, and some only operate under the name and not a practice name. ? There will also be some duplicates which is fine. ? Where there are multiple practitioners at one practice please enter in at 2 or 3 of them.
