I have an existing laboratory request form inside my Electronic Medical Record that needs to be revised. The current layout no longer captures everything our clinicians want, and a few fields are now obsolete. Cost needs to be under $50USD as this is a one page document to revise I will supply: • A 1 page PDF export of the form as it appears today • A marked-up version showing the fields to add, remove and reorder • Access to a staging copy of the EMR (web-based) so you can develop and test without touching production data Your task is to implement those changes directly in the form builder/templates used by the EMR, verify that the new form stores data correctly, and ensure it prints cleanly to PDF. Once finished, I need a short note of what was modified so I can keep our internal documentation up to date. Experience working with medical-record platforms, HL7/FHIR field mapping or similar web form engines will help you move quickly, but it’s not mandatory as long as you are comfortable digging into existing template code (HTML, Liquid, or proprietary tags). Turnaround is flexible for the right person, but ideally I’d like a testable update within a week. Let me know if you have relevant examples or need clarification on the environment and I’ll get those details to you.

Hi. The risky part here is changing the form layout without breaking the EMR’s underlying field mappings or stored data. I’d treat the marked-up PDF as the UI specification, but first trace each existing field to its template/builder key and persisted value so obsolete fields can be removed safely and reordered fields continue mapping correctly. I’d make the changes in staging only, verify create/edit/save behavior for every affected field, then test the rendered PDF for page breaks, labels, spacing, and missing values. I’m comfortable working through unfamiliar template code and existing web systems without unnecessary rewrites, and I’d provide the requested change note after validation. Should removed fields be permanently retired from the underlying data model or only hidden from this form? Retiring them requires checking historical-data dependencies; hiding them preserves existing records and minimizes schema impact. Best, Kara.

Hi, We can update your EMR laboratory request form while keeping the existing data flow and functionality intact. Our approach: • Review the current form, PDF export, marked-up requirements, and existing template/form-builder structure. • Identify the fields to add, remove, rename, and reorder, including any related data mappings. • Implement the changes in the staging environment without affecting production data. • Verify that each updated field stores and retrieves data correctly through the existing EMR workflow. • Check required fields, validations, formatting, and compatibility with the current form logic. • Test the completed form with representative entries and confirm the generated PDF is clean, readable, and correctly aligned. • Review for any unintended impact on existing fields or workflows. • Provide a concise change summary for your internal documentation. We can work within your requested budget and aim to provide a testable staging update within the desired timeframe. Regards, Chanchal CnEL India

Hi, I can handle this update within your budget. I’ll work from your existing PDF and marked-up version, make the required additions/removals/reordering directly in the EMR staging environment, and verify that the updated fields save correctly and the final form prints cleanly to PDF. I’m comfortable working with existing HTML/template-based forms and tracing how fields are stored, so I can adapt to the EMR’s current form builder or template structure. I’ll make all changes in staging first and provide a short summary of exactly what was modified for your documentation. Price: $50 USD Testable update: 1–2 days Once I can see the marked-up form and know which EMR platform you’re using, I can get started. Thanks, Yasir

★★★ EMR Form & Template Specialist ★★★ A lab form can look correct on screen and still break the EMR underneath it if a field is removed without checking what its ID is mapped to. I’d treat the existing field mappings as the source of truth, not the PDF layout. I’ll start in staging by tracing the current fields to their template definitions and stored values, then apply your marked-up add/remove/reorder changes in the EMR builder or HTML/Liquid/proprietary template. For obsolete fields, I’ll determine whether they should actually be removed or simply hidden/deprecated so historical records remain intact. After the revision, I’ll submit test records and read the saved data back to verify each changed field reaches the correct destination. I’ll test the PDF independently as well, checking page breaks, spacing and print-specific CSS rather than assuming the browser layout will print correctly. You’ll receive the tested form plus a concise field-by-field change log for your internal documentation. I can keep this within the $50 budget for the one-page revision, assuming normal staging/template access. Send the staging access and marked-up PDF, and I can begin.

Which EMR platform is being used? Is the form configuration-driven or template-code based? Will database schema changes be required, or are the new fields already available?

Hi, I am Adeel. I built Code3Scribe, an EMS documentation platform with HIPAA compliant storage and structured, protocol driven forms, so working inside a clinical record template with strict field rules is familiar territory. I would start from your PDF and markup, rebuild the field set in the staging template, then test data capture and print output before touching anything live. Once the fields are confirmed I would document exactly what changed for your internal records. Can you tell me which form builder or template engine the EMR uses under the hood?

Hi there, I see you need to revise your laboratory request form to better suit your clinicians' needs. With your provided PDF and marked-up version, I can implement the necessary changes directly in the form builder used by your EMR. I have experience with HTML and template coding, which will allow me to efficiently manage the fields you want added, removed, or reordered. Moreover, I will ensure that the new layout functions seamlessly during data storage and prints correctly to PDF as requested. To refine our approach, I have a few questions: 1- What specific fields are you looking to add or modify? 2- Are there any particular design considerations or constraints you want me to follow? 3- Do you require any additional documentation aside from the modifications summary? I’d be happy to discuss details to implement a solution that meets your needs effectively.

Hi, I'm Denis, a full-stack developer with experience on healthcare systems involving form management and data workflows. I understand your need—a straightforward update to your lab request form that captures the right data and produces clean PDF output. The core work involves identifying your form template structure, making the marked-up field additions and removals, reordering the layout, and validating data storage and PDF printing. This is straightforward once I understand how your form engine handles field definitions and template rendering. I've handled similar projects requiring work within existing template systems, reverse-engineering field mappings, and testing across output formats. The key is understanding whether your EMR uses standard HTML templating, a proprietary tag system, or Liquid, then making changes in the right place to avoid downstream issues. My approach: review the PDF export and marked-up version you provide, access the staging environment to locate the form template, implement changes directly in the form builder, test the data flow end-to-end, verify PDF output, and document all modifications for your team's tracking. One upfront clarification—are there field dependencies or conditional logic in the current form that might be affected by reordering? That determines our timeline. A week is realistic for this scope. I can start immediately. Let's discuss the details. Thanks, Denis.

Hello!⚡ Thank you for listing the sections you need. I would be glad to build this template for you. I recently built an Advanced PDF quote template in NetSuite with cover and profile pages, brand fonts and a line item table that held together across page breaks. How I will run it: 1. Build it as an Advanced PDF template in FreeMarker against your real quote records, not a mockup, so field behaviour is correct from the start. 2. Lay out with tables rather than flexbox or grid, since the BFO renderer behind Advanced PDF supports only a subset of CSS and modern layout collapses silently. 3. Set the cover and company profile pages so the running header and page numbering begin at the quote itself. 4. Upload your brand fonts to the File Cabinet and declare them in the template, and reference images so they still render for external recipients rather than falling back. 5. Line item table with headers repeating across page breaks and the totals block kept from splitting. 6. Test against long descriptions, many line items and missing fields, then document how to adjust each section yourself. Thanks. Hassan

Hi, The part most likely to bite here is the reorder, not the add/remove: EMR form builders usually bind each field to a stored key, so moving or renaming a field can silently break the mapping and the PDF print layout even when the screen looks right. I'd work only inside the lab request template on staging, keep existing field keys for anything that survives, retire the obsolete ones without deleting historical data, then submit a test order and check both the stored record and the PDF export against your marked-up version. You'd get a short change note listing every field key added, removed or moved. Which EMR is it, and are the templates plain HTML/Liquid or a proprietary tag set?

I've built and revised medical forms in Epic, Cerner, and custom EMRs before, so this is right in my wheelhouse. I'll review the PDF and markup, then map the changes to the staging EMR's form builder—likely Liquid or HTML templates—while ensuring HL7/FHIR field alignment. I'll validate data storage, PDF rendering, and edge cases, then document the changes concisely. I can start immediately. Thanks, Andrii.
