Add a page to my web-site, c#o#m#p#u#-#p#r#o#.co#m (removing the “#”s), for the “Contact Us” tab, to be a copy of my home page, except as follows:
1) Put a checkbox next to (top left of) each logo on the page, including cloud-Utilicy logo.
2) Put a checkbox next to (left of) each bulletted item on the page, replacing those bullets.
3) Replace the sentence below each heading, “Consulting”, “Systems Engineering” and “Software Development” with: “Check all that apply…”
4) Reduced in size the cloud-logo in top-right to make plenty of room for #5, #6 & #7.
5) Add text to say, “Tell us about your needs…”, and “Check all that apply…”.
6) Add Input boxes in top-right for three fields prompted by labels…
b. “Name”, and
c. “eMail Address or Phone Number”.
7) Add a “Send” button which does the following:
a. Enables only when one or more checkbox is checked, or there is text in in the "Comment/Question" field.
b. Sends the list of checkboxes checked and fields (#6) via your choice of any one of the following methods:
i. FTP Append
ii. Google Docs append to a spreadsheet
iii. Append to a .txt file in the S3 bucket hosting the site
iv. eMail to an address
c. Displays a “Thank you” dialog box and returns user to the site’s home page.
• The web-site is hosted on Windows 2003 Server.
• You will be provided with a destination address for #7 above after you choose method.
