Hi, Here is what I understand of your requirements: - Page 1 shows list of products / items (say 5). User can check those products which he wants to rate. Lets assume user checks 2 products. - Next page asks for user ratings for the first product checked. On pressing next, page asks for 2nd products's rating. - Third page: survey ends and an email is sent to the system specified address. Here are few questions: - What questions are there on the survey for each product? Only one? Or multiple questions? - Do you have an existing site where this should be integrated? - Should the answers be saved in db? - Should user need to log in or identify them self? - If data is to be saved, do you need back end pages to view the survey results? thank you, Nirandas
