Online Quoting Tool

This project involves building a website that involves four 'minor' take on forms (e.g. user take on) and three 'major' quotation take-on forms, combined with reporting and database integration. The major take-on forms would have as many as 30 different fields and multiple product input lines. Also, the major take-on forms need to have the capability to upload a bill of materials through a pre-defined spreadsheet. This bill of materials would append to any data already input. Edit and modify capabilities would be needed. Within the take-on forms there are a number of calculations that must take place as data is entered, the most simple of which would be 'value x qty = total'. All fields within the quotation take-on forms would need to captured in a back-end database. Quotation forms would need to be emailed to clients with any relevant 'appended' documents. Multiple reports based on quotations generated would need to be developed. The entire 'model' has been created in MS Excel and has thoroughly tested, including all form layouts, fields and formulas for the calculations. I would need clear guidance on what development tools to use as well as the best choice of database. PURE GRAPHICS WEBSITE DEVELOPERS NEED NOT APPLY. The PRIMARY skills needed here is 'online form development and database integration'. Applicants would need to provide examples of similar type websites. Completion required by end-Feb 2013 (latest).
