Created a fillable PDF version of the Uniform Residential Appraisal Report ( Fannie Mae Form 1004 / Freddie Mac Form 70 ). The look of the form is exactly like the original one, and all fields can be filled using any PDF Viewer ( Adobe Reader / Foxit PDF etc. ). After filling, the text of the form can be saved, sent by email or sent to database server. An image of half of the first page of the fillable PDF Form is attached.
