This is a screenshot of the Windows version of an app that charted child development. The wedges in the "wheel" could be clicked to set different development levels. The box in the upper-right could be clicked to change from a solid color fill to various shape patterns that represented level of impairment. Thre was also a version of the app for Mac OS X.
