I have a word document that I would like converted to PDF.

However, the complexity is that:

1. I would like all 'answer' fields converted to editable PDF fields that you can tab between etc and all tickboxes to be able to be 'ticked'. I hope this makes sense.
2. I would like the general layout and aesthetics of the document to be improved and beautified.

I have attached the .doc that needs to be amended. I have also attached a .pdf as an example of the finished product i am looking for.

The first person to do this properly will be awarded.

Can you please make the form spaces are white, the text all black, and the colour scheme & layout reflective of the professionalism of the example I attached. Orange etc.
