Hi. I am purchasing a template like this:

1. I want the color scheme for the site to be similar to this: enjoymaids.com
2. This is a maid service that gives back to charity. So I want the theme to be a maid service but I want it to highlight we give a portion to charity.
3. I want the top to include "Book Online Instantly", Gift Cards, Help and Contact
4. I want a map of the United States and I want it to highlight the areas we serve. Only highlight the state of Georgia. I attached an example but want it to look different than what is attached.
5. I want my design to have a video next to the slider like http://www.maidsinblack.com/ Just put the space for the video, I will create replace it later.
6. I attached an example of what I want below my header where it has social proof.

Just to clarify, this is a homecleaning/maid service. A portion of proceeds will go to charity so want the design to look vibrant. I am purchasing this template http://demo.themestreet.net/launch27/happyslider.html so this is somewhat of the format as well as the examples I included.
