Need this done fast. Will award prize as soon as someone completes these simple tasks.

Do 4 things related to jQuery datepickers that reside on a web form:

1) fix CSS bug in #date-of-birth jQuery datepicker. It doesn't look correct: see site.
2) set range of #date-of-birth field to 1900 - 2010
3) add a datepicker input field to "Please specify when your first chemotherapy infusion is schedule for?" aka #infusion-start-date
4) set range of #infusion-start-date, to today-5 years from now
