I need a clean and modern redesign of this VERY old Google Adwords Proposal. Here's a link to the old proposal:

The intent of the proposal is for the sales rep to print this out and bring it to the prospect.

I'd like to see updated chart design and screenshots. Also, I need a spot for each of the following:

--Company Logo
--Company Phone Number
--Company Web Address
--Head-shot, name, title, phone and email of sales rep
--Small paragraph above sales rep information that reads: "An effective pay-per-click advertising strategy is pivotal to a company's online success. I'd love the chance to share some success stories from a few of our other clients. Feel free to contact me using the information below."
