Write an Android application

Create an Android and/or iPhone application exactly like the Health Risk Assessment Survey with an Adobe PDF output results page available at [login to view URL] using their 7-day free trial account. It must be in a format which can be submitted successfully to both the Android and iPhone Stores for free downloads. It must also include a backend ecommerce capability for customers to pay for the results after they take the Health Risk Assessment Survey. Each responding Freelancer can only bid on either the Android or the iPhone application and not both. You must be able to show me 2-3 other Mobile Apps you already developed similar to this Survey Question and Response format. You must also create an Admin back end available from a Web Site using either ASP.NET or ASP 3.0 (preferably) to allow me to edit both the Questions the possible Answers and the Rating Scale applied to each Answer. The back end must be a Microsoft SQL Server database (nothing else). You should use standard Microsoft development tools for this project.
