Task is to :

Visit the location and get form filled sign and stamped by Sohar Hospital.

Take 3 to 4 geotagged photos with name board visible

In case of refusal to fill form take written or verbal record of refusal with Name, Designation and Contact of the verifier.

In case Place not found take 4 to 6 photos of surroundings with 1 minute interview of person nearby confirming this place does not exist.

If the above requirements fulfilled then task will be considered complete.
