Mobile website data entry to feed searchable online database

We require a mobile web-based database or a smart phone/tablet app for our employees to fill in their daily hours and tasks onsite.
The data provided via the smart phone or tablet, will feed information into the online database that can be accessed by admin staff from our office.
We have up to 20 workers across multiple construction sites that need to provide their individual hours for specific tasks each day.
The work categories, tasks, description and hours will be provided by the operator.
The information will be provided to us daily, and populate a database that can be viewed online.
At the end of the working week, each individual employee should receive a summary of the information they have provided for each day via email at the end of the week.
The online database should be able to be searched and categorised into dates, employees, sites, tasks, etc. so that the admin staff can easily locate the information they require.
We would also like the option to export the data to excel if possible too.
The phone/tablet access doesn't necessarily need to be an app, it can just be a phone/tablet friendly webpage link with a few fields to be selected/entered by the user.
