Write Simple VBA Form Questionnaire

I require a VBA form to be created within an Excel Workbook that will ask the user a series of questions and store their answers in a database. The questions allow the user to design partitions of different specifications based on the included Workflow diagram and an unlimited number of partitions should be able to be created and edited from one form. I have included the Workflow for the questionnaire with comments and also .a basic form design you can use. Not a very complicated design or requirement, however, I am mainly use it in order to find a capable programmer to outsource other work. Any applicants should be able to code in VBA but ALSO Visual Basic. I have clients who have solutions in both VBA and VB and I am looking for a programmer to work with who can work well in both. All applications should have an estimate of the time required to complete, please be advised that I am a software developer and am aware of how long that this job takes. Part of the requirement I have is that I can trust someone to give decent evaluations of the time it will take them to perform projects and that these fall within my estimates. I have much more in the pipeline that I would like to outsource to a capable individual so look forward to working with a capable developer on ongoing developers going forward.
