Multiple choice voting grid.

I am looking for a multiple choice voting grid. This system will let people choose either /or and give me the results in and email
The selections will be made by radio buttons so you can only choose one of the choices per question. And the grid will notify you if miss a question.
Selections will be similar to
Cats vs Dogs
Red vs Blue
50lbs vs 70lbs
350 jellybeans vs 500 jellybeans
The users email and phone number will be collected via a free form box and
results will be emailed to me via an excel file.
The voting system will also include a timer ticker so everyone knows when the voting will end.
lastly, when the results are emailed to me I would like it to be pre-formatted and sorted by column A, column B, Column C so the results will be easy for me to read.
