Design New Pages for existing Website

We are using existing website template for our product and need some one professional to design and code new pages based on our requirements. You can get familiar with our template by visiting [login to view URL] We would like to establish long term relationship with some one that is good with UI/UX and hopefully HTML/CSS as this is on going effort. When we need new page following information will be provided: 1. Sample of existing page that needs to be converted in case it already exists; 2. Description of what needs to be shown and what that page should be. At the end we need to get new page that will seamlessly integrate with our design. For example we will need all pages necessary for full authentication flow. Not only what is already provided, but additional to that Email Confirmation, Two-Factor authentication related pages etc.
