Design a Nameplate That Can Be Used as an Overlay in Adobe Premiere

Good afternoon all you fantastic freelancers out there! I'm in desperate need of a freelancer to design for me a nameplate overlay I can use when editing my videos. It needs to be animated and make use of my logo in some way, shape, or form.

I want the nameplate to be animated as stated before, for it to be easily overlapped while editing my videos, and for there to be two name bars similar to TGN's and The Know's.
Included with this I would like for you to type up a set of instructions such as how to change the names inside of these name bars, sets a duration for the nameplate again, and so on.
