Need something super simple that anyone can use.

1. Customer will fill out 15-20 items. ie. business name, address, his/her name, tel number, email address, car info, date, time, etc.

2. Once completed, customer can provide payment ie. credit card info. and when/if approved, he/she will get an immediately print out of a PFD ticket/form.

The PDF ticket/form must have the info customer provided and our company name and contact. Also a ticket number that is unqiue for each ticket. I will provide the ticket numbers, there is a special sequence that must be followed. Also depending on what the customer put in, the price total will need to be calculated.

it would be nice if the customer can look for past orders, past ticket/form,

I don't have a website or hosting, but can purchase one right away. Ideal person can send me a mock up or example before going forward with me getting the hosting plan.

Credit processing - ANYTHING but Paypal would be acceptable.
