Create Javascript code to upload picture on existing website

I am using an existing website that is not created by myself. I need some javascript written to upload files from the local machine. So let says I am uploading something from a earlier specified path, example C:\[url removed, login to view] it will uploads on the form and then I can fill the rest out with javascript that I am yet to create myself and then submit the form.
