Write Iphone and paired Apple Watch Application

Requirements:
Looking to create an Iphone app and paired Apple watch app.
-When App is engaged on Apple watch or Iphone, the app should use the accelerometer in the Applewatch to tally the number of times a certain type of movement is performed.
-Users tally's should be counted and available to review on the Iphone like the Iphone health app. See tally's from day, week, month, etc. avg/ day etc.
Fairly basic app but am looking for a freelancer that has decent graphic design skills as well to make the app, icon, etc appealing from a visual perspective as well.
Please include examples of previous work.
