Zoho/Database functionality extension

I need the below functionality and features to be added to an existing client Zoho Database. If this is not possible I will require a detailed explanation of how else these requirements can be achieved.

I need the successful bidder to READ THE REQUIREMENTS CAREFULLY, address each section outlined and explain to me how this can successfully be accomplished using Zoho.

I need to be able to understand the process before we proceed.

If these requirements can not be achieved using Zoho I am also interested to understand why. You are invited to put forward in your quote a system that can accomplish the below requirements, explaining the system and the necessary tools such as MySQL, PHP etc.

IMPORTANT: Please be clear on the process you intend to use, and outline a realistic time frame and price for this project.

NOTE: I have included a screenshot ([url removed, login to view]) of the current form fields that will also need to be included/further developed.

REQUIREMENTS

1. Third table to include the ability for Admin to enter more specific notes and save them under the current date, these fields are to including:

- Date of consultation

- Case notes

- Further information

- Name of Caseworker who visited with the patient on that particular day

- Case/Progress Notes (unlimited in characters)

- External Referrer: Name of person or agency that referred the patient

- Every record is to include automatic date of entry and last edit

These notes need to be segregated by dates. This means a drop down is necessary for all the dates that have entries in them. The Admin user can then select a relevant date and have all the notes taken on this day visible.

2. Need to segregate out user access. This variant user levels with variant degrees of system usability need to be assigned. These user levels need to be broken down as follows:

- Admin - with full system access

- MID access which will be broken down into individual logins that need to be tracked in regards to history of login and what changes were performed by this user.

- Reception level access - this is a generic login with access to only certain fields within the system and needs to be locked out of others. The fields Reception login will not be able to see or edit will include

- Date of visit

- Nature of enquiry

- Last caseworker to see the patient

- Source of income

And all other fields can be visible or editable by reception.

3. The system needs to be capable of uploading of large files, at least 15 - 20 megabytes.

4. Customise the database/Zoho to fit in with the Client template including:

- Imagery

- Colour thyme

- Fonts
