OSC Customized order form

We are need of a freelancer with excellent OSC skills. We have 2 items that need to be addressed. 1-Fix the current bug that has error when you go to checkout. 2-Add customized order form for one catagory of products we have on our website. We will furnish you a photoshop file, but will need you to write the code the fields you see on the order form that has been upload. Keep in mind we will need the standard order form checkout form for all other catagories. Should you have any questions please email or PM.
