Multi Web Submission Form and Responder with Database

We are in need of custom script interface / web based program for our website to be used exclusively to accept multi-part applications online from potential clients. Would prefer to build as an independent script but interface with [login to view URL] to be able to track affiliate/reseller referrals. Specifications to be as follows:MySQL, PHP, ASP, .cgi,other ??? 1)Multi page forms that can be managed and modified directly from a browser based control panel by authorized admin or users. 2)Admin must have the ability to add, edit, change or modify the following from the control panel: a)create custom web forms with single or multiple pages. b)ability to mark fields as required and force the user to complete these fields before being allowed to continue to the next step / page. c) Issue a UserName & Password that is unique to each applicant as they begin the form submission process so that they may return at a future time to complete their application. d) Capture all partial data in the form even if the user closes the browser before completion of the form. e) A checklist of required documents that must accompany the application - they may log in at a later date and add the documents to the application area that was created when their application began. f) An automated email reminder system that has the ability to read the applications database and determine whether the application is complete or missing documents - at that time the system would send an email reminder out to the applcant letting them know what part of the application or required documents they are missing to complete the application. ** Must have a scheduler built in or cronjob tabs to be able to change the frequency of the reminders as well as a series feature to be able to set up custom timed warnings and reminders to send out at different schedules determined by the admin. g) .html interface that allows admin to modify headers , footers and arrange fields from within the cntrol panel or web based editor. 3. Affiliate links / applications - Would like to create affiliates within the application panel that Admin can assign an affilite ID unique to a reseller or affiliate to track referrals to the applications page made by resellers - Would prefer to use a module that would track all referrals and enter them into : [login to view URL] 4. Ability to export the application data to a .csv, excel, and or populater a live .pdf application form ready for download or attach as an email and send to Admin when completed. *** the admin should have the ability to open and view all documents and applications and chose to expor, populate, delete, or edit - as well as move to folder or assign toreseller or affiliate within the IdevAffiliate program. NOTE** If you have suggestions on another affiliate tracking script that we could use I am open to suggestions - All applications must run with secure certificate on our server & domain. Backup: Script is required to do autmatic backups of databse and configuration files as well as system restore feature built in.
