MS Access DISASTER Form

Currenlty, I have a word doucument that captures data for DISASTER VICTIM IDENTIFICATION FORM. I need to transfer this Word Document into an Access Database where I can do the following:

Add/Delete/Edit & Report Functionality to New and Old records

(2)

Needs a "Creative" form built, so the end user can update the information in Time Critical Situtaions

(3)

Consolidation of data elements into drop down list.

(4)

Support post deleivery for bugs

This project needs to be completed by 12:00 p.m. on Tuesday to receive payment.

## Deliverables

1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Installation package that will install the software (in ready-to-run condition) on the platform(s) specified in this bid request. 3) Complete ownership and distribution copyrights to all work purchased.
