PDF/FDF CGI Script

I have a pdf file that contains fill-in forms and checkboxes. I need someone to create a CGI script that will handle the data in FDF format to either populate a Mysql database or e-mail the form data to an e-mail address.

I have a pdf file that contains fill-in forms and checkboxes. I need someone to create a CGI script that will handle the data in FDF format to either populate a Mysql database or e-mail the form data to an e-mail address.

This must be able to run on my server.

The coder must be familiar with this process already. I am not willing to work with coders that will "take a crack" at something they've never done.
