Web Form Revamp with Quickbase Database Backend

This is the first part of a larger project that needs to be completed a week from today. This first part is smaller and will show my client that you can do the work. Please read the following carefully: We currently have a web form that needs to be revamped. The form upon submittal post the answers to a Quickbase database using the Quickbase API. The revamped form needs to: 1. Layout the form questions over multiple pages and sections. 2. Enable navigation to a given section (see attached "MTW Form Layout") 3. Save the results to a Quickbase database over multiple tables. For example 1 question asks for a "client name" and another asks for "Employer". The "Client Name" is stored in a "Client" table and the "Employer" is stored in a "Employer" table. 4. With each form submission an additional record will be needed to be added to a status table. In another words the form will not provide the data for the status table. The data for the additional status record will be based on applying a few rules to the other form answers. 5. Though this form currently resides in a Drupal website, this form can be stand alone on something like Wordpress. 6. Provide save and resume functionality such that the form can be partially filled out and completed upon revisit. 7. The form layout needs to match the page structure of the attached "MTW Form Layout Part 1" pdf. The page design doesn't necessarily have to match the "MTW Form Layout Part 1" pdf. It would be nice if it could match the website look and feel but a simple clean look is sufficient. Project Approaches The approach you use is up to you as long as you accomplish the above goals by 8/5. Here are the possible approaches. Approach 1 has the advantage of all requirements are there except for the save and resume. 1. I am attaching for you the existing website code files "[login to view URL]". It may be easier for you to modify these files where much of the coding is done. This existing form code already does most of what is asked above. All that would be required is to: a) change ordering of questions to match "MTW Form Layout Part 1". b) separate the form questions into pages following the "MTW Form Layout Part 1". c) Change CSS format accordingly d) Introduce the save form and resume functionality. 2. Use the attached HTML code "HTML Form Code" that produces multiple forms which collectively represent all of the questions needed for the revamped form. Here you would need to do the following changes: a) Combine the code to produce 1 form to match the page layout per the attached "MTW Form Layout Part 1" contained in the attached zip file . b) Add functionality for save and resume. c) Change CSS format accordingly c) Using the Quickbase API, automate the creation of an additional record to the Status table. 3. Create your own coding to accomplish all requirements as described above. Project Deliverable You will provide the web form files into the ftp directory: such that I will be able to see the web form in a browser. I should be able to test this form by entering in the information for a person. Stopping and coming back later to my computer to resume my session without needing to re-enter the answers already provided. Then checking the Quickbase backend that the web form answers have been recorded appropriately. Our timing on this work is immediate.
