WEB Form using GOOGLE Map API to check if address is in city

I need a WEB form that takes a CITY and / or ADDRESS as separate inputs, displays a map with the city outlined, a place marker for the address and displays a note if the address is within the city or not.
If just the CITY is entered, display just the outline of the city.
If just the ADDRESS is entered, display map with address centered.
If the CITY and ADDRESS are entered, display outline of CITY with place marker for address and a note if the address is within the city (or not).
Needs to use Google MAPS API.
I have attached a simple form as an outline.
