Frontend and Backend PHP Project (repost)

This is a simple PHP project for someone who knows what they are doing and knows PHP properly. I need a PHP web programmer to make a web page (which contains a form) for me, which replicates another web page on another site. The other site has split this form into many different pages whereas I need it all on one page. Once the form has been submitted, it will collect payment from my payment processor along with all the information and enter it into my MySQL database. Then I need a backend which will allow administrators to view and manipulate this data as well as submit part of this data to another website at the touch of a button. The backend will have LOTS of functionality and it has to look sleek and professional. It has to have a nice layout and be very user friendly with sort features and many of the functions you find in software today. The backend will also have payment processor integration. Ajax will probably be needed in certain places. If you can do this and can do this quickly and at the lowest cost, then this will be a deciding factor. I have been quoted $120 by a couple of programmers and firms. But price is secondary to actually being able to do this quickly. There are many PHP Programmers here on RAC so I'm sure you realize the competitiveness. You will have to work on my site so I can test as you work. ## Deliverables
1) Complete and fully-functional working program(s) online.
2) Deliverables must be in ready-to-run condition, as follows? (depending on the nature? of the deliverables):
a)? For web sites or? other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software? installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).

Should work on all browsers without any negative or adverse affect.
