Interface/forms creation for web based Intranet Portal(repost)

I am currently developing a web based intranet application and I need someone to build the forms and interfaces for the application. I am looking for someone with experience in creating really lovely web forms and also someone with AJAX experience. I currently have 10 forms and pages from the application I want to build so please send me sample links if you have any.
