JQuery/HTML/CSS work for UI demo/prototype

I am producing a locally-runnable HTML prototype of a fairly complex web-form. I did the rapid prototyping portion of the project using [login to view URL], which gave me rudimentary show/hide control using YUI framework.

I would now like to take this form off formstack and jazz it up using JQuery UI and any other JQuery code/plugins.

I will provide the raw HTML from formstack output and a specification of the flows through the form.

## Deliverables

The form is currently implemented on [login to view URL], which provided a rapid way to test business logic.

I now need to re-implement the form as a JQuery enabled form for a more high-fidelity demonstration using:

jquery core

jquery UI

jquery forms

?? any recommended plugins to speed development time.

*The following **form** enhancements will be required:*

1. autocomplete (I have a list for the data)

2. masking for dates, phone numbers, currency

3. validation for email address format

4. optional date picker

5. required field validation (inline messaging)

*The following **UI** interactions will be required:*

1. Show/Hide form elements based on previous selection (possibly effects added - fade/ease

2. Lightwindow effect to load large explanatory text blocks from external data file or from in the document

3. Add/append DOM elements - in some cases the workflow allows to "add additional" addresses or people. This may be managed by just hiding a maximum of 5 additional copies of a field set, or by using javascript to dynamically create a new instance. I don't have a preference. Faster completion time is the key.

4. For/each add logic. Users pick one or more items from a list which require subsequent details to be added on the next screen for each one selected.

Thanks everyone for your questions and bids. I will re-iterate that this is a front-end-only project. My requirement is that it be runnable on a laptop potentially without internet connection. To clarify questions that have been asked: 1. The demo I produced at [login to view URL] may be used as a starting point. Ideally, the html source should be cleaned up to remove the table-based layout but this is not a 'must-have' 2. Formstack uses YUI for it's javascript interaction. I requested JQuery mainly because I'm more familiar with it. However, if everything can be done using YUI, or if you want to use a combination of YUI & JQuery, then that's fine. 3. Depending on timing, there will be an additional piece that might use an external call to a 3rd party address validation web service that returns XML. This is not in-scope for this project but if there is time at the completion of this project, I will offer the additional work to the successful bidder. Thanks everyone.
