Affiliate ID customization script

I'm looking for a script that relates to affiliate ID's. I run an affiliate program and I would like set up article redirect pages where an affiliate can just type in their affiliate ID (I use Clickbank) in a form at the top of the page and the links on the page automatically all turn into their affiliate link. In addition, I would like to have social share buttons on each of those article entries so that an affiliate can immediately just click a button and post the article link through a variety of social sites, e.g. Facebook, Twitter, Google, etc. And it should be their affiliate link that they just created that gets posted. This is a sample page that will show you what I'm looking for: [login to view URL] I have some knowledge of coding, so once I get the functioning script for a sample page or article section, I can apply that to the rest of the article entries myself, so that would be the extent of the project.
