Electrical cost calculator per KWH

We need an electrical cost calculator programmed that will allow people to estimate their average electric costs based on size of their home (square feet), how many people live in the home, what things they leave on, an average time they leave them on.

Example:

How Many Sqfeet Is Your Home: [ ]

What City, State do you live in: [city], [state]

Type the quanity and average hours used per month for the following:

- Computers: [quantity] [hours]

- TVs: [quantity] [hours]

- Lights: [quantity] [hours]

- Refrigerator: [quantity] [hours]

- Oven: [quantity] [hours]

- Microwave Oven: [quantity] [hours]

- Dishwasher: [quantity] [hours]

- Coffee Maker: [quantity] [hours]

- Washer: [quantity] [hours]

- Dryer: [quantity] [hours]

- Clock: [quantity] [hours]

- Radio: [quantity] [hours]

- DVD CD VCR : [quantity] [hours]

- Furnace: [quantity] [hours]

- Cieling Fan: [quantity] [hours]

- Space Heater: [quantity] [hours]

- Window Air Conditioner: [quantity] [hours]

- Hot Water Heater Use (for showers, baths, etc.): [quantity] [hours]

The calculator should base the calculations on watts of each appliance * hours of use to ge the KWH (see [url removed, login to view] for example). The watts of each appliance is saved in a database table. It should then multiple the resulting KWH by KWH cost which is saved in a database table. The KWH cost is stored in a MySQL database table for each state & city. So the calculator should query this table by city/state the user input in order to get the KWH cost. It should then calculate the KWH used * KWH cost and output the total cost on the screen.
