PHP Script Depositing Data in MySQL

This is a small project requiring knowledge of PHP Scripting for gaining access into a MySQL database which will be hosted with a well known hosting service. The MySQL basic table structure will be prepared by me. The hosting server does not allow virtual access and therefore a short php script needs to be prepared as "wrapper" or "executable" or "dash-q" to be used within other page language to guarantee that the collected data from a php based form web page is dumped in the MySQL. This project is part of a published but not yet promoted web based business. The intent is to offer monthly paid membership by allowing paid members to access non-published web pages as long as their membership is current for the month. This project is divided into two (2) parts: A. The front part that will work with a php form page that will collect the prospective member's basic information also allowing for the setting of a username and password. Data from this form will be deposited in the MySQL. I will prepare the php form page, you will prepare the php code that will be integrated with the rest of the page and deposit the collected data in the MySQL. B. The script that allows access by a member by quering the MySQL by username and password. It should also include the ability to use the server based PHP Email to assist the member with a forgotten username and/or password. This is expected to be completed by you and owned by me. I am not a programmer but I have enough knowledge and require complete explanation of the project's working. Once this project is satisfactorily completed and tested, I am considering automation which will create more work for you. This being my first time, I have no idea what the budgeting requirements are. Please bid and I will consider all bids based on a review of comparable projects. I do require quality.
