simple MMC snap-in

Hello dear coders, NOTE: Should be done in Visual BASIC 6.0 I have a university task to make a simple MMC snap-in template that should perform only two functions - show date and a button that will display a form with my information (text). So what I really want to have is a snap-in that can be added to mmc. Please bid only if you can provide what I need. It should be an easy task for a person who wants to get excellent rating. I also need a brief explaination on how to do it.
