Unique Wordpress Theme

I need 1 original Wordpress theme for my blog. Theme design should be creative, and unique 2 or 3 column, adsense and SEO optimized. Navigation should be along the right side, and Pages along the top. I would like a JPEG header that I can change and update with little difficulty. I do not have images yet, so original header graphic will need to be created and should be related to my content.

I will also need an opt in point at the top of the page to one side that will be used to download an ebook I have written.

I will also require that all PSD files be sent to me in a ZIP folder that I may use for editing. I will upload the theme to my website myself.

Information regarding my website content and details will be given at time of selection.
